Case Management

Consultants Grace Stevens, Scott McPherson and Tracy Cocks are experienced at managing the full continuum of vocational rehabilitation activities for individuals with a broad spectrum of medical issues including chronic pain, musculoskeletal and soft tissue injury, disease and complex psychiatric cases, to facilitating workplace reintegration. Our team of consultants is focused on optimizing the outcome of every case while ensuring timely case resolution. Consultants are able to access and utilize medical information, are knowledgeable regarding supports and services available in the community and are part of a strong network of service providers and related professionals. Consultants are able to liaise with the individual and their health care team, insurer and employer to develop vocational rehabilitation plans and goals in accordance with the vocational hierarchy of objectives.

Early Intervention

Vocational Rehabilitation Consultants Grace Stevens, Scott McPherson and Tracy Cocks along with Occupational Therapist Joanne Vanderhorst provide individualized early intervention programs, which are proven to help claimants remain at work or return to work sooner, resulting in less lost time and improved productivity. Early intervention services may help an employee to remain at work during treatment and recovery, or to assist an individual to return to work while still on short term disability. Following assessment and a review of current medical information, CCI’s Consultants will develop a vocational rehabilitation plan with recommendations. Recommended supports and services may include referral to specialized treatment programs, community services or occupational therapy. Vocational Rehabilitation Consultants will also develop, implement and monitor medically appropriate Remain at Work/Return to Work plans with timeframes and objectives and will ensure that barriers are identified and addressed without delay.

Return to Work Plans

Consultants Grace Stevens, Scott McPherson, Tracy Cocks, and Joanne Vanderhorst are able to plan, coordinate and implement customized programs that take into consideration the needs of the individual as well as employers and insurers. Regular monitoring and reporting ensure that potential barriers are identified and addressed early. Gradual return to work programs may be used to ease the individual’s reintegration, allowing for earlier return to work at reduced hours, while continuing participation in off-site treatment and gradually increasing to full-time hours. Consultants are able to work closely with the healthcare team to ensure that employees return to work quickly and safely and to facilitate the accommodation process when indicated.

Field Visits & Home Assessments

Grace Stevens, Scott McPherson, Tracy Cocks, and Joanne Vanderhorst will conduct home visits and 1, 2 or 3 point assessments as requested. Reports provide an overview of the case based on information gathered from the client, care team and/or employer. Our Consultants are skilled at identifying and providing information regarding critical medical details, barriers to recovery or vocational rehabilitation, cost projections and vocational rehabilitation strategies with timelines leading to return-to-work. Reports are customized to the insurer’s requirements and may include recommendations regarding medical management and/or services and supports.

Motivational Interviewing

CCI Consultant Grace Stevens is trained in Motivational Interviewing, and skilled at eliciting behavior change and overcoming resistance using a directive, respectful, communication process. Motivational Interviewing is a directive, client-centered approach that leads to a trusting, therapeutic relationship that supports positive change.

PGAP (Progressive Goal Attainment Program)

PGAP is an evidence-based treatment program for reducing disability associated with pain, depression, cancer and other chronic health conditions. PGAP is considered one of the most empirically supported interventions for targeting psychosocial risk-factors for disability. To date, clinical trials have supported the effectiveness of the PGAP for reducing disability and promoting return to work in individuals who have sustained work injuries, individuals with whiplash injuries, individuals with fibromyalgia, and individuals with mental health conditions such as depression and PTSD. The following trained PGAP providers are able to assist their clients in overcoming the challenges associated with debilitating health/mental health conditions through this Program: Joanne Vanderhorst and Grace Stevens.

Functional Improvement Programs

Provided by Joanne Vanderhorst and Grace Stevens, Functional Improvement Programs are customized to meet the individual needs and circumstances of the individual and the disability insurer; These programs provide structure and encourage an individual to re-engage in meaningful life-role activities including exercise and activities outside the home. Consultants meet regularly with their clients to plan activities, monitor progress and provide a level of accountability.
